Different Types of Rhinoplasty

Which Rhinoplasty is Best For Me? 


Rhinoplasty, also known as nose surgery, is one of the most common types of cosmetic plastic surgeries performed. This, however, is hardly surprising since the nose is something that is constantly display, dead center in the middle of your face. It's no wonder this surgery is so often frequented. 

So what type of nose surgery is best for me? You'd be surprised, not everyone has the same nose or needs the same surgery. There are so many different types of nose surgeries, you'd be amazed. 

First, there's the bridge. The bridge can be flat, big, wide, have a bump, or have deviation. It may be too thin or it may be too bulky. The tip, it can be too short, or it may even be too bulbous or too long. As for the wing of the nose, otherwise known as the alar, they may be too wide when you smile or too round.

Aside from these surgeries, there are many surgeries that a plastic surgeon must take into consideration.


Reduction Rhinoplasty
A majority of patients seeking rhinoplasty surgery are generally concerned that the size of their nose is disproportionate with their facial features. 

Areas of particular concern are the presence of a bump on the bridge of the nose, excessive width of the bridge tip, and possibly the width of the nostrils.

In the picture to the right, you see that this patient only underwent alar plasty: reduction of the nostril wings.


Augmentations Rhinoplasty
Augmentation rhinoplasty is basically the surgery where the dimensions of the nose are increased.

This is probably probably the most frequented nose surgery by Asians since their nose bridge tends to lack bridge and dimension. In this case, an implant may be used to add height and dimension to the area that is needed.


Ethnic Rhinoplasty
In essence, all rhinoplasty procedures are ethnic rhinoplasty since we must take into consideration the ethnic structure, and characteristics of the shape, along with the skin and facial proportions.

For example, Asians may prefer a more soft, delicate nose frame to go along with their soft facial features, where as a caucasian or middle eastern may prefer a more strong nose lone to go with their stronger facial features.

Post-traumatic Rhinoplasty
This procedure is for those who have undergone facial trauma. Injuries to the nose frequently lead to damage in the bone and cartilage support, which could lead to a change in their appearance due to deviation, assymetry, or problems in the air way.

Deviations of the bones and cartilages of the nose, can be reset with relatively good success rate

Who Needs Bulbous Nose Rhinoplasty?



People who have a bulbous nose find that their nose is very low, wide, and blunt. The nose is the center of our face, so having a slim, projected tip can give our overall impression a much more sophisticated touch. Those who have a blunt tip, may want to slim and redefine their tip. Depending on the bridge height, you can do bridge augmentation + tip plasty together for your ideal result.




A bulbous nose can be divided into 3 types and the surgical plan is different depending on your nose structure.



First Type of Bulbous Nose: Stubby Tip

If your tip is stubby, nasal tip cartilage needs to be bound and projected. 

Second Type: Wide Alars
Tip projection would not be enough to achieve your ideal look if you have wide alars. Though patients at GNG Hospital initially consider only bulbous nose correction, they usually end up deciding to get alar reduction as well. The rims of the alar are removed and the shape of nostrils is also corrected.



Third Type: Bulbous Tip + Wide Alars

Mostly, patients who are considering bulbous rhinoplasty have both a bulbous tip and wide alars together. Thus, in most cases of bulbous nose reduction surgeries, patients generally decide to undergo both surgeries for a higher satisfaction rate!

Rhinoplasty Without Artificial Implant at GNG Hospital! -- NoPlant Nose Job

Rhinoplasty Without an Implant?

In Korea, silicone and goretex is widely used for its sleek, perfect finish. In photos, especially, the nose can look perfect in every angle. Silicone can be a great option for patients with low height and/or minimal definition. However, in the case that the patient has an adequate aesthetic height, there's no actual reason for an implant. To thin the nose, the bones do need to be broken, but this can be done in a way that would not require a subsequent dorsal implant.

So, what are the options?

We can harvest cartilage from either your nasal septum or possibly the ear, then graft it into the bridge, eliminating the need for a nasal implant. In the case that the patient doesn't have enough of both, then donor cartilage will be used instead or fused with the other materials. Another popular option can also be cartilage extraction from the patient's rib. Through advanced methods and

What is NoPlant Rhinoplasty?

NoPlant Rhinoplasty uses dermis and bionic prosthesis instead of artificial prosthesis. Usually, dermis tends to be absrobed as time passes.
Since NoPlant Rhinoplasty uses fine ground diced cartilage powder wrapped in fascia, the possibility of absorption is very low and shock resistant. The bridge line is very natural and people will hardly notice you did rhinoplasty!

Another case when NoPlant rhinoplasty is needed is when medial osteotomy or rasping is needed for a hump nose. Since the bones cannot be cut or rasped down perfectly straight, diced cartilage can be used to fill in the grooves of the bridge for a more smooth, sleek flow from bridge to tip - mimicking that of an artificial implant.

This method is also popular to patients who are considering revision after a botched nose job or implant rejection.

Why You Should Never Use an L-shaped Silicone!


L Silicone VS. I Silicone Implant



L-shaped implants are quick and easy to use when projecting the overall height of the nasal bridge and tip. With half the surgery time, the patient can get an easy projected (taller) line linked from bridge to tip. They were used in the past due to the easy, simple handling and reduced time in the surgery room. However, with the advancement of technology and methods, the I implant has come a long way to help surgeons design a more natural nose line from bridge to tip.

Some issues with the notorious L implant include:
⟹ Pressure from the corner angle of the L causing extrusion or necrosis in the tip ⟹ Bulbous or stretched nose tip ⟹A Pointy or unnatural line and tip ⟹Redness and/or thinning of skin ⟹Higher risk of infection ⟹Unsatisfactory elevation/balance of the dorsum or tip

The idea of the shape of the L implant may have been a good idea, but it has not achieved the results that were intended.


As you can see in the photo above, the skin above the implant doesn't remain the same. The skin becomes thinner and thicker along the bridge while the corner angle of the L has an uneven amount of skin and fat tissue around it.

Strengths And Weaknesses of Nose Implant



Different Implants Used in Rhinoplasty 👃



🌀Silicone
Many people are the most familiar with this method. This type of implant is the most suitable for people who want a perfect nose line flow, especially in photos. More common among females.

📌Strengths
1. Low chance of rejection
2. Shape doesn't change and strong against heat
3. It has elasticity and flexibility
4. Easy to sculpt a defined line
5. Easy to remove if there is a problem
.
📌Weaknesses
1. It can be obvious in thin skin
2. There is a chance of movement and is vulnerable to shock



🌀Surgiform (goretex)
Surgiform is a compressed goretex. Original goretex is easy to get compressed so it makes nose bridge lower overtime. Ideal for people who want a more strong, straight line. More common among men.

📌Strengths
1. Able to achieve natural line
2. Since it's a compressed version, the shape changes less than original goretex
3. It's strong against shock
4. It's not visible even in thin skin
5. Almost no rejection

📌Weaknesses
1. There is a chance of slight shape change (but less than goretex).
2. Can take longer to remove if there is a problem



🌀Noplant 4th generation
If the patient is uncomfortable with artificial implant, then noplant 4th generation is a good choice. Noplant is made of diced cartilage wrapped by fascia.

📌Strengths 
1. No rejection
2. Able to achieve natural line in both genders
3. Minimal absorption rate compared to just using dermis
4. It's good for revision surgery since it's strong against infection
5. It's strong against shock
.
📌Weaknesses
1. There might be a slight change of shape due to absorption
.
Each implant has its own strengths and weaknesses and can be decided after a face to face consultation with the doctor. The best implant can be decided based on the patients’ skin type, nose structure, and desired style

What goes in and on your nose after rhinoplasty?

So what exactly goes in and on your nose after rhinoplasty?


1. External cast: the external blue cast will be molded over the nose immediately after surgery to maintain nasal bones in their new position. Kind of like a helmet to protect and secure! 

2. Internal cast: also known as Silastic, is a bendable, flexible plastic nasal stent used to protect and secure the internal structure of your nose.

3. Nasal packing: After surgery, nasal packing will be inserted to soak up any immediate discharge, blood, Mucous, etc. following surgery

4. Stitches: the blue stitches will be removed on the 7th day post-op along the columella and possible alar region. White dissolvable stitches will be used along the inner inside of the nose; they will dissolve within the next few weeks

5. Depending on what the doctor recommends, many of few options can be considered for bridge augmentation: silicone, goretex, rib cartilage, donated rib cartilage, and/or fascia graft. Fat or filler can be another option as well.



🌀External + internal cast, stitches: 7th day removal
🌀Nasal packing: 2nd-3rd day removal
